Registered Nurse - A licensed health care professional, who has completed a 2-4 year training program. RN’s are case managers, who work hand in hand with physicians to create and edit the patients care plan. RN’s are involved with all phases of patient care; teaching, and assessing to name a few.
Certified Diabetic Educator (CDE)- A CDE promotes excellence in diabetic education, through the development, maintenance, of diabetic programs offered for the diabetic. The CDE credential has become a standard of excellence for the delivery of quality diabetes education, CDE; Lois Laktas will assist you with understanding diabetes.
Certified Home Health Aide - A caregiver who has received special training so as to be able to provide personal patient care; bathing, grooming, nail care, bed linen changing, basic food preparation, reminders to take medications, basic homemaking necessities, under the supervision of the Registered Nurse.
Physical Therapist - A professional trained or certified by the state or accreditation agency to design and implement physical therapy programs. These programs are generally designed to promote strength, coordination and safety.
Occupational Therapist - A licensed health professional that is trained to provide training in activities of daily living, use of adaptive equipment, energy conservation and fine motor coordination. A plan will be designed with the possibility of using assistive devices and adaptive skills. Emphasis may be on increasing ability of dressing and bathing, self care, cooking, housekeeping and leisure skills.
Social Worker- Social workers provide mental health services for the prevention diagnosis and treatment of mental and behavioral illnesses. Their goal is to protect and maintain their patient's function.
Speech Therapist/ Speech –Language Pathologist - A therapist who evaluates and treats communication and swallowing disorders. Speech therapists often work with patients who have suffered a CVA.
